Job Duties/Requirements:
- Perform patient care within the scope of licensure, following Institution/Facility policies, procedures, and evidence-based Physical Therapy practices.
- Provide Physical Therapy treatments and evaluations on an as needed basis as scheduled by the Institution/Facility CEO, CME, or designee.
- Deliver Physical Therapy services to patient/youth populations based on:
- Physician referral and order,
- Direct access requests for an existing and documented diagnosis.
- Conduct initial assessments/evaluations for each referral and develop a treatment plan aligned with the findings.
- Communicate treatment plans clearly to the patient/youth and relevant medical staff.
- Complete all reports, assessments/evaluations, and progress notes using approved documentation forms within the patient chart or outpatient health record.
- Remain available for telephone consultations while on duty at the Institution/Facility.
- Maintain daily charting; no chart may remain delinquent for more than 10 days.
