AvaMed Inc. is a leading Healthcare Staffing Agency specializing in placing qualified providers throughout California State Correctional Facilities. We are currently seeking a Physical Therapist for a full-time assignment at the correctional facility in Stockton, CA.

Monday - Friday 7:30am - 4:00pm
Weekly Pay
$85/hr-$90/hr


Job Duties/Requirements:

  • Perform patient care within the scope of licensure, following Institution/Facility policies, procedures, and evidence-based Physical Therapy practices.
  • Provide Physical Therapy treatments and evaluations on an as needed basis as scheduled by the Institution/Facility CEO, CME, or designee.
  • Deliver Physical Therapy services to patient/youth populations based on:
  • Physician referral and order,
  • Direct access requests for an existing and documented diagnosis.
  • Conduct initial assessments/evaluations for each referral and develop a treatment plan aligned with the findings.
  • Communicate treatment plans clearly to the patient/youth and relevant medical staff.
  • Complete all reports, assessments/evaluations, and progress notes using approved documentation forms within the patient chart or outpatient health record.
  • Remain available for telephone consultations while on duty at the Institution/Facility.
  • Maintain daily charting; no chart may remain delinquent for more than 10 days.
